PACE stands for Program for All-Inclusive Care for the Elderly. It is authorized by New York State (Medicaid) and CMS (Medicare). FHW-PACE provides insurance coverage, medical care and social support to participants who may otherwise be in a nursing home, but they wish to remain at home in the community.
